Recent ATLAS and CMS measurements have reported an excess in the ttbar production cross section near threshold, which may be related to the formation of a ttbar bound state. This has renewed interest in the theoretical description and simulation of threshold ttbar production at colliders. After briefly reviewing the experimental status, I will discuss some of the current theoretical approaches, highlighting open issues and challenges.
